Requirements of Duplicate Category A or B (Damaged or Worn Out Cases)
- Application for Duplicate Category with reasons
- Two (2) colored photocopies of CNIC
- Three (3) specimen signatures duly attested
- Four (4) Photograph Passport size
- Surrendering the damage / worn out Registration Certificate in Punjab Pharmacy Council
- Pay Order / Demand Draft for issuance of Duplicate Certificate Fee in the name of Secretary, Punjab Pharmacy Council of Rs. 5000/-.
- Registration Copy if available
- Original Registration File
- Personal Appearance in Punjab Pharmacy Council / Appearance certificate from Area Drug Inspector (if necessary).
