Change of Name
Requirements of Duplicate Category A or B (Change of Name)
- Application for Duplicate Registration with reasons (Change of Name)
- Two (2) colored photocopies of CNIC
- Three (3) specimen signatures duly attested
- Four (4) Photograph Passport size
- Four (4) Color photocopies of degrees of B. Pharmacy, Pharm – D reflecting the change of name
- Fee for Degree verification as prescribed / required by the University Concerned.
- (Degree Verification fee Challan concerned university)
- Pay Order / Demand Draft for issuance of Duplicate Certificate Fee in the name of Secretary, Punjab Pharmacy Council of Rs. 5000/- .
- Surrender of original Registration Certificate
- Personal Appearance in Punjab Pharmacy Council / Appearance certificate from Area Drug Inspector (if necessary).
All required documents attested by the concerned University.
